Unstaple and sort before you start
A staple lifts the corner of every page underneath it, which puts a shadow across the top-left of each capture and makes edge detection guess. Pull the staples, square the stack, and put it in the order you want the PDF to read.

Capture pages in sequence without stopping
Shoot page one, two, three straight through in one session so the PDF order matches the paper. Keep the phone at the same height throughout — pages captured at wildly different distances look inconsistent and make a long record tiring to read.

Go easy on lab results and charts
Result tables, reference ranges, and ECG or growth-chart grids are exactly what heavy contrast destroys. Keep the adjustment light so the faint grid and the small units next to each value survive. If a page came out dim, relight and recapture rather than pushing the slider.

Handle thermal and faded printouts first
Pharmacy slips and some machine printouts are on thermal paper that keeps fading, and pages stored in a hot drawer degrade fastest. Scan those pages before the rest, and if a page is already too faint to read on paper, it will be too faint in the scan — request a reprint from the provider.

- Scan all the fronts in order, then flip the stack and scan the backs, then use page reordering to interleave them. Or simply scan front, back, front, back as you go — slower, but nothing to reorder afterwards. Skipping blank backs is fine, but check that they really are blank.
- Can OCR make my records searchable?
- OCR turns printed text into selectable text with auto-detect plus Latin, Korean, Chinese (Simplified), and Japanese scripts. Clinical documents mix printed and handwritten content, though, and handwritten notes, signatures, and margin annotations will not be recognised reliably. Do not treat OCR output as a complete record.
- Are scanned medical records accepted by a new doctor?
- Many clinics accept a legible PDF for reference, but for anything that affects treatment they will typically request records directly from the previous provider. Requirements differ by clinic and by country — ask the receiving office what they need rather than assuming a scan is sufficient.
